You will have received a Welcome email that included the paperwork requiring signatures. Please complete them and return via email, HIPAA-compliant fax, or US Postal Service. All contact information is listed at the bottom of this page.

Preparing For Your Appointments
In-Person Session

While I provide a cup of cool bottled water, you are welcome to bring your own beverage.

My office is located on the 3rd floor of the Victorian Gardens Building at 6918 Shallowford Rd, Chattanooga, TN - Click here for directions

Parking is in front, side, and behind the building, and there are no steps to enter the building.

For your privacy, the door is marked as suite 317, with no other identifying information. However, if you were to forget the suite number, there is a directory board at the elevator.

When you enter the suite, a bell will chime to make me aware you have arrived. Please enjoy the peaceful waiting room. Keys to the hallway restrooms will be on the table. You are welcome to use our LCSWGuest WiFi (password is beourguest)

Please do not knock. I will greet you when I am ready to begin your session.

Payment is collected at the beginning of the session; and with scheduling being completed online, none of your session time is used for administrative tasks.
